Healthy children, as well as hospitalized and chronically ill children or children with disabilities, often benefit from a team approach. To efficiently serve our patients, we request that you fill out the referral form attached and email it to discoversmiles@gmail.com or fax it to our office at 517-574-5894 before the patient arrives at our office. If your office has digital Xrays, please email them or send them via mail.
